Iker Casillas has been set a March deadline by his doctor to establish whether the legendary goalkeeper should continue playing or retire.
The 38-year-old, who lifted the World Cup with [/news/spain/index.html Spain] in 2010, wishes to resume his career at Porto, but his time on the field will likely be brought to an end this year against his will.
Casillas suffered a heart attack in May last year, and began training again in November.

But despite those close to Casillas insisting that the Real Madrid legend is recovering well, there is no certainty that he will recover to the extent where he could take part once again at the highest level of the sport.
And it is this dilemma that could convince Casillas to explore other avenues after hanging up his boots, with the goalkeeper currently acting as the link between Porto's squad and management.
The health of his wife, Sara Carbonero, who was diagnosed with cancer last year, is also crucial to deciding Casillas' future.

His decision to remain in Porto for the last season was due to health problems suffered by the pair.

According to both, remaining in the city is the ideal scenario for their recovery, as they are able to enjoy more privacy than they would typically be afforded back in their home country. 
Both have even pondered remaining in Portugal even if Casillas decides to retire from football.
The Spaniard has also already been presented with several offers, specifically from LaLiga, and there is a possibility that Casillas could become president of his native football federation.
However, Casillas has promised Porto president Pinto da Costa that his priority will always be with the Portuguese giants  - and if he decides to hang up his boots, then Porto will be his first choice to start life as a director. 
But Casillas will not choose to return to Spain should be continue playing, and instead is likely to move to MLS. 
Casillas will ultimately make the final decision on his career in March, but the stopper has admitted to those closest to them that the most obvious choice will be to retire.
